Payments

Please plan to take care of co-payments, co-insurance and any outstanding balances at the time of service.  Co-payments or deductibles must be paid at the time of visit.  We accept debit cards, Visa and Mastercard.

If we are not a participating provider with your insurance plan, or your provider does not cover services provided, you will be responsible for any charges incurred.  We will provide you a receipt for your insurance company.    

Any check returned from the bank will result in an additional $20 charge.

Always notify our office of any change in name, address, phone or insurance information.

We realize that emergencies do arise that may affect timely payment of your account. If such extreme cases do occur, please contact a patient account representative at (801) 546-0131.

Self Pay Policy

If you do not have insurance we require a $100 payment for a first time visit.  All appointments thereafter will cost $75 and must be paid at time of service.  A 25 percent discount will be applied to all billed charges.

Insurance

Our staff will make every effort to simplify all billing and insurance claim procedures. We participate with many insurance plans; however, we recommend you call your insurance company prior to your visit to verify coverage and to determine if a referral from your primary care physician is necessary. Please bring your insurance card with you to every visit so that we may ensure that our records are accurate. If you do not have insurance coverage, full payment is required at the time of service.

Co payments, co-insurance, and any outstanding balances are expected at the time of service. Patients may be financially responsible for payment of all services even if their insurance company does not pay. Patient accounts not paid promptly are subject to third party collections and/or legal procedures.
